What it usually means
Reverse flips the load on the brake pads — they shift to the other face of their carriers, so worn pad hardware, missing anti-rattle shims or slightly loose caliper pins clack in reverse first. A morning groan in reverse is often just overnight surface rust wiping off the discs — normal if it clears in a stop or two.
The park-brake mechanism dragging can also mutter in reverse on some cars.
